Change of details
Submit when you would like to advise the school of any change of details (e.g address, phone numbers, emergency contacts etc).
Application for extended leave
Submit application for an absence of five days or more at least two weeks prior to the commencement of planned leave.
Special religious education & Special education in ethics participation
Submit when you would like to change your SRE/Ethics option at any time.
2026 Atypical return to school advice form
To be completed by parents or carers of students who:
(A) will not be present at Cherrybrook Public School on the first day of school, Monday 2 February 2026, or
(B) will not be returning to Cherrybrook Public School in 2026.
